  • Human error blamed in horrific elevator death of ad exec last year

    The elevator accident that killed an advertising exec in December was due to repairmen forgetting to re-engage a safety mechanism after working on the lift hours before, a city probe has found.

    Suzanne Hart, 41, was killed when she stepped into an elevator at 285 Madison Ave. on Dec. 14 and it shot upward with the doors still open, trapping her between the elevator and the shaft wall and killing her.

    According to the investigation by the Department of Buildings and Department of Investigation, the workers disabled a safety mechanism that prevents the elevator from rising with its doors open - common during maintenance - but didn't put it back into place.

    The DOB has suspended the license of Transel Elevators, which was serviced the units at the building, and will try for a full revocation of the license at a future hearing.
